The power of Payroll Giving

I authorise the above deduction to the RBWH Foundation to occur on the frequency specified above.  This nominated amount will be deducted every fortnight from the first available pay after the receipt of this authorisation and this will continue until withdrawn by me. I authorise Queensland Health Payroll Transactional Services to amend the arrangements for the processing of payroll deductions and nominated fixed amounts from my fortnightly salary and wage payments in accordance with my instructions above.

I understand that the RBWH Foundation will send me updates about their activities, the projects they fund and their fundraising appeals and handle my information in accordance with the Privacy Policy.  

I consent to the publication of my name on the RBWH Foundation Payroll Giving ‘Honour Roll' (as published on RBWH Foundation marketing channels).
